New User Registration

Any insured benefits included in the UCA Membership are NOT MAJOR MEDICAL or COMPREHENSIVE HEALTH COVERAGE and is neither Minimum Essential Coverage under the federal Affordable Care Act (ACA) nor is it Workers’ Compensation Insurance under state law. The insurance benefits are limited and include a pre-existing medical conditions limitation.

It is important that you review your fulfillment materials for details on your benefits and your responsibility when submitting a claim.

If you have any questions or concerns, please call 800-290-3607 (Monday- Friday 9am to 5pm EST) to speak with a representative from the Member Services Department, or emailsupport@mymemberinfo.com.

It is your responsibility to view your plan details. You have 30 days to view your benefits to be certain they fit your needs. If you are not completely satisfied, you may cancel your plan and receive a refund within the first 30 days.

You will receive a refund in the first 30 days (provided that you have no paid claims).

Helpful Tips

• Please use the primary member's Date of Birth.
  The person who is the main plan holder.
• On desktop devices please format the date of birth like this MM/DD/YYYY.  Example 06/01/1980.
• Make sure there are NO blank spaces in username or password.   Blank spaces can result in processing errors.
